Article 45

The notice of appeal shall contain:

(a) the designation of the appellant as a party to appeal proceedings in accordance with Article 2, and, where the appellant has appointed a procedural representative, the name and address of the representative;

(b) the file number of the decision against which the appeal is lodged and a statement as to the extent to which amendment or cancellation of the decision is sought.
